On June 13, 2007, Michael D Ford sent in a form to the Secretary of State in Texas to change the address of the Dr Roger E Marks Healthcare Foundation from 1021 Holden St (which is the address of the Glen Rose Medical Center) to 4590 Highway 56 South. The original Certificate of Formation had the Glen Rose Medical Center address. Note that the registered agent did not change in this filing and as of February 7 2009, Mike Ford is still listed as the registered agent on the SOS website.
