Puzzled about City of Glen Rose Citizen of the Year Award (2016)

I don't live in the city, but in the county, so I have no specific skin in this game, could not have voted one way or the other for anyone, but am an interested observer. I have fairly regularly over the years been to City of Glen Rose meetings and recorded them, as most of you know. During that time, I have never seen multiple people win "the" Citizen of the year award. I have no opinion as to whether any person has deserved to win.
But I do wonder why, since the form that went out to all the people of Glen Rose in their utility bills says "The City of Glen Rose seeks to honor one person.... " that multiple people won the award this last time. I asked the City if there is a set of rules that are followed, and got back a copy of the form that was sent out. I had also heard that only 20 people out of more than 2200 people that live in the city actually voted, and that the winners got 5 votes (both people apparently listed on the same slipls, ie X and X).
So here's my questions.
1. Does everyone receive this form? What if a person has direct deposit and doesn't get the bill?
2. If someone had gone in to pay their bill at the city office, did the city personnel remind them to vote?
3. Since there were 2 people on 5 slips (presumably those people didn't understand the clear rules on the forms?) why didn't the city draw straws or do something to ensure there was only one winner?
4. Suppose some people in the future decided to game the system by going around and asking that multiple people be put on a slip of paper, so that there was again multiple people winning (one person jokingly told me, what if a dog was nominated and voted in by the right number of people).. Would the city then revert back to insisting, as they say on the form, that the winner is *one person*?
5. Did City Council persons vote on this and decide that there should be multiples?
Again, I am not weighing in on whether the winner was apprppriate or not. But the process seems broken to me.
Update: As per comments below, there is one other time that a group of people, the volunteers at the United Methodist Thrift Shop won the award. Plaque of all winners of the award is hanging inside by the front door.
If the City wants to nominate multiple, people, businesses, or charitable organizations, then it should be clear on the form. For example, suppose in this round, people wanted to nominate the people who generously donate their volunteer time to maintain the greenery/flowers around the square. Or suppose someone wanted to nominate multiple people from another business, such as, say, Green Pickle or Glen Rose Auto Parts. The people would assume FROM THE FORM that that was not the intent, that there should be ONE person nominated rather than multiple people from a business. That is unfair because it sets two different standards for how the citizen would be chosen.
